Use the Sports Registration Form below to sign up for the
Molasky Community School NFL Flag Football league.
(One form per participant required.)

To print a copy, follow these simple steps...

1. Click on the Sports Registration Form below to open it 'full-size'.
2. Click File at the top-left of your browser and select  Print...
3. Select your printer from the list and click the [Print] button.
4. Once printed, close that pop-up window to return here.

Once you've filled out the form, fax it to Molasky Community School at (702) 655-4856. You can then contact Sheena about how to pay.
